synchros on fourth stuffed? Or it sounds like an issue with the gear itself maybe.

gear box is fcuked! - the teeth on the 4th gear have worn away so it slips out everynow and then when its not fully in or you fast shift it

thats what happens when manual drivers like to fast cluth and dont have their stick all the way into there desired gear

im not sure if you can actually fix that unless u change the gear box altogether
my old 1990 celica had that problem it used to pop in and out
