Wouldn't be a chain, would be a blower belt.

I've also heard of ring gears coming off flywheels & inflicting damage..

imola 94,

pedro lamy in a lotus slammed into the back of jj lehto in a benetton, sent a wheel flying into the grandstand injuring 9 people and killing 1 guy.

then 7 laps later senna died... and the day before roland ratzemberger died, and the day before that barrichello smashed into the wall and broke his nose!
