Sluggish and restricted fto

Hey,
Lately I've noticed my Fto is running slow and feels very restrictive specially between 2.5 - 3.5k rpm it's feels like it gets stuck between those range. Overtaking becomes me putting my foot down nearly flat! And also sometimes I'd hear a note change in my exhaust (no I'm not taking about mivec).
Any ideas?

Fuel filter, spark plug, leads were all changed 15,000kms ago.
but even before then the problem was there.
im thinking it could be the exhaust system?

O2 sensor or injectors?
*For my information, around what km range does the injectors need attention?*

I did intend on cleaning the injectors (but cbf to take the plenum off).
How often does the o2 sensor need to be replaced?
Also when I was under the car I noticed there's 2 o2 sensors! One was in the cat and the other before the cat.
Why do I have 2 and which one do I need to change?

The one on the cat is the cat temp sensor... the one in the Y pipe is the O2 sensor.... I think the O2 sensor only really needs to be replaced if you have a very rich mixture and shaithouse fuel economy...
Honestly i just realized why the hell did I speculate on the O2 sensor when your problem sounds like engine hesitation

Yeah don't think it's my o2 sensor as my petrol economy is great =D

Was reading something on mivec.co.uk about the timing belt, if it's out by even 1 tooth it can cause performance and pick up to slack off a bit. He did a before and after dyno and it was a different of up to 40-60hp just from that. So, how many kms have you done ad has the timing belt been changed recenty?

Blocked catalyc convertor, Could it be a sticky throttle body? or slack accelerator cable?

Ok I so I had a look at the cat today and its safe to say the cat is dead! It's hollow haha!
Ok now since my muffler is new, how can I clean the internal of the exhaust? I'm assuming that's where the rest of the cat is and chlausin some blockage
