Tachometer not reading properly

Tachometer not reading properly
As the title states my Tacho is not reading correctly, and the Revs are completely wrong. When I floor it the revs actually go off the meter, and just spins around. Has anyone else had this problem, and if so how did you fix it?

I have a taco problem as well, but mine is just dead...
